Starting June 1, 2003 the new Universal Voucher Form will be available on the Financial Services website for FY04. The form then can be completed online and printed out for signatures.

The new voucher form will replace the following forms: The Confirming Requisition Form, the Reimbursement Request for Emergency Purchases Form, the Request for Payment Form, and the Honorarium Payment Request Form.

ALL the above forms will be discontinued effective July 1, 2003 and the new Universal Voucher Form MUST be used at that time.

The new Expense Statement, Petty Cash, and Working Fund Forms have also been updated and posted on the Financial Services website. Please begin using these forms July 1, 2003.

The new E Travel will also be posted shortly to the Financial Services website. Please continue to use the current form for all FY03 travel and begin using the new form for FY04 travel.
