1. Co-Sponsorship Application (Tutorial 1 and Tutorial 2)
2. Meal Pre-Approval Form (Tutorial)
- If food will be purchased for event
3. Liability Release Form
- If event will be off-campus or is a high-risk activity
- Examples: Paintball, Skiing, Rock Climbing, Bowling, etc...
***Within 10 Business Days After Event Please Submit***
2. Sign-In Sheet of all who attended
3. Copy of Email or Flyer that advertised your event
